I’m seeking for a job right now but I don’t know the sequence of the materials when I submit them: 1. Cover Letter; 2. Resume; 3. Letter of Recommendation / or I should put the letter of Recommendation on the top?

I manipulated a Microsoft Word template to include the following:
FAX (in large bold print – is this really necessary it is part of the template?)
Left side of table:
To: (their name)
Fax: (their fax number)
Re: (name of open position)
Pages: (don’t worry I counted the cover sheet)
Right side of table (so my name is across from the “To: name”)
From: (Me)
Phone: (my phone)
Date: (the date, duh)
I deleted the comments part, do I need to include that?…
Does anyone know how to incorporate your salary requirements into a cover letter. I am a college student and I want to start sending my resume and cover letter out to employers so I can obtain a job when I graduate this may. Believe it or not none of my professors ever taught this in any of my classes. I have been searching online for templates but I can’t seem to find anything useful can anyone offer any advice.

The template that I am using says “Dear _____,” to begin. I could leave this off but it seems informal and like I am mass emailing facilities to get a job.
Most of the places I am applying to say to send a resume to an email address, so I don’t know the hiring person’s name. Should I just put Dear HR or Attention: HR or something?

Right now, I’m applying for an undergraduate non-profit internship for an International NGO specializing in Human Rights. Instead of focusing on my extracurricular and work experience like I traditionally do, I’d really like to emphasize my coursework this time. I’m applying to work at Build-a-Bear Workshop (in Canada) and they told me to bring in the application form with a resume and cover letter.
I’m 16 and looking for my first job so I have no work experience. I also don’t have any “academic achievements” and I’m not involved in any clubs or sports at school. (I’m in grade 10 by the way)
